Family Health Clinic
Family Health Clinic exists to ensure affordable, culturally-appropriate, high-quality health and wellness services. A staff of physicians, nurse practitioners, nurses, nutritionists and case workers provide quality care to infants and children birth to 21 years of age living in Warren County and surrounding counties and adults age 21 years of age and over living in Warren County. Services Provided for infants, children and adults:
- Acute Care visits
- Maintenance visits (hypertension, diabetes and other chronic diseases)
- Physical Exams: These include Routine physicals, Well Child checkups, Sports Physicals, Work Physicals, Pre-Operative and Head Start physical exams
- Immunizations
- Hemoglobin testing
- Urinalysis
- TB skin test
- Development Screening
- Referrals to appropriate agencies (if ordered by provider)
- Lead testing for children
- Prevent Blindness program for assistance receiving free eye exams and eye glasses
- Car seat program
- Medication Assistance Program
Service Fees:
- Adults age 21 and older must provide proof of residency and proof of income on first visit
- Charges and balances figured on a sliding scale according to household income and family size for those uninsured
- Co-pay of at least $15.00 at every visit for uninsured adults
- No co-pay collected at time of service for those under age 21 who are uninsured
- Medicaid, Medicaid Managed Care plans and most private insurances accepted
- At this time, we cannot bill Medicare for services

Vaccine Fees are subject to change at any time. Insurance will be billed as applicable. Please contact your insurance provider to verify coverage of the vaccine fee. The Vaccines for Children (VFC) program is available for qualifying children. The fee is $15.00 per injection for VFC vaccines(included in Fee below).
